NeedLink Nashville is only able to assist with past due balances. Please wait until you have received a past due bill with a cut-off notice to apply. If your balance is current, your application will be denied, and you will be instructed to apply again once you have received a past due bill. 

All applicants should be notified of their application status with a phone call within 10 business days of applying.

In order to complete this application, you will need:

  • Your NES account number, the name on the bill, and the service address
  • Income Documentation
  • A Government Issued Photo ID
  • If your name is not on the account, you will need the account holders photo ID and signature. 

If you do not know your account number, please contact NES to obtain it before submitting your completed application.

  • Paycheck
  • Unemployment Verification Letter/Separation Notice
  • Proof of Government Assistance (SNAP, MDHA, Families First Verification Letter, etc.)
  • Social Security, SSI, or SSDI Verification Letter
  • W2, 1099,1040 Tax Forms
  • Letter from Employer stating anticipated wages
  • Section 8/HAP Letter
  • Pension/Retirement Statement
  • Signed Statement certifying zero income

You are still responsible for paying this bill and for making future payments.

I understand that filling out this application DOES NOT guarantee that I will receive assistance and my utility bills are my responsibility.

NeedLink Nashville may request information, share information, or work with creditors and assistance providers on your behalf.

I authorize NeedLink Nashville to discuss my application with my utility company, housing provider, creditors and their representatives, case manager, anyone listed on this application, and any other government, business, church, or nonprofit representative with an interest in resolving my financial situation or providing me with additional services.

NeedLink Nashville may use written, verbal, or electronic communication to obtain account details, notify creditors of pending payments, coordinate social services, verify information, and obtain documents needed to process this application.

Non-identifying information and client stories may be shared for community engagement, newsletters, fundraising, and reporting purposes.

Release of Liability

To the fullest extent permitted by law, I hereby release and forever discharge, and agree to indemnify and hold harmless NeedLink Nashville, its officers, agents, employees, and volunteers from and against any and all liabilities, claims, demands, or causes of action of any kind on account of any loss, damage, illness or injury to person or property in any way arising out of or relating to my application from assistance and/or related activities, whether due to negligence, mistake or other action or inaction of NeedLink Nashville or any person or entity acting on its behalf.

By providing your signature below, you are agreeing that the applicant has permission to apply for financial assistance with the NES account under your name and understand that no other NES account in your name will qualify for assistance for at least 6 months.

The Impact Nashville Assistance Network, hereinafter referred to as "CharityTracker", is a shared, computerized record keeping system that captures information about people experiencing need for emergency services, including but not limited to assistance with utility bills, medications, rent/mortgage payments, etc. United Way of Metropolitan Nashville (Administrating Agency) administers CharityTracker on behalf of participating agencies of the CharityTracker Assistance Network, including NeedLink Nashville (Participating Agency).

I understand that all information gathered about me is personal and private and that I do not have to participate in CharityTracker. I have had an opportunity to ask questions about CharityTracker and to review the basic identifying information, which is authorized by this release for the CharityTracker Assistance Network Participating Agencies to share. I also understand that information about non-confidential services provided to me by CharityTracker participating agencies may be shared with other CharityTracker Participating Agencies. This Release of Information will remain in effect for 3 years from the date noted under my signature at the bottom of this page unless I make a formal request to this Organization that I no longer wish to participate in CharityTracker.

I authorize NeedLink Nashville, as a CharityTracker Participating Agency, to share my basic, identifying and non-confidential service transactions/information with other CharityTracker Participating Agencies. I authorize the use of a copy of this original to serve as an original for the purposes stated above. I further authorize NeedLink Nashville (Participating Agency), as a CharityTracker Participating Agency, to share my dependent's basic, identifying and non-confidential service transactions/information with other CharityTracker participating agencies.
